The LPN will provide direct patient/resident care under the supervision of clinical leadership and support daily nursing operations, including:
Administering medications and treatments as ordered
Monitoring patient condition and reporting changes
Documenting care accurately and timely
Supporting admissions, discharges, and transfers
Following infection control, safety, and facility protocols
Communicating professionally with patients, families, and care teams
Active LPN license in Pennsylvania or eligibility to work in PA
Recent LPN experience preferred
Experience in LTC, SNF, rehab, post-acute, or nursing home care is a plus
Strong bedside care, documentation, and communication skills
Reliable, professional, and assignment-ready
